Summary of the Rights and Responsibilities of Organizations
in the Process of COA  (Re)Accreditation

This summary of rights and responsibilities applies to all organizations engaged in the COA (re)accreditation  process, including public and private  organizations, employee assistance programs, opioid treatment programs, and financial management and debt counseling services.  For more detailed information regarding an organization's rights and responsibilities, please refer to COA's Accreditation Policies and Procedures Manual and your Accreditation Agreement.

Right:      

To receive this summary of your rights and responsibilities at the time of application for (re)accreditation.

Right:

To withdraw from the (re)accreditation process at any time.  COA (re)accreditation is a voluntary process, and all organizations have the right to withdraw from the (re)accreditation review process  at any time.

Right:

To receive  courteous, collegial, timely, and facilitative service from COA.  COA staff is available by phone, toll-free, at (866) COA-8088, Monday to Friday, 9:00 AM to 5:00 PM, EST.  The COA web site, provides  current information on (re)accreditation and includes a  list of staff e-mail addresses and telephone extensions

Right:

To receive a  set of COA's current and applicable standards, as well as its  Accreditation Policies and Procedures Manual, which fully and completely describes  all aspects of the (re)accreditation process.

Right:

To communicate a concern or complaint  to COA concerning any part of the (re)accreditation process or related services, to file a grievance against a Peer Reviewer, and/or to appeal a (re)accreditation decision.  These procedures are fully described in COA's Accreditation Policies and Procedures Manual, which is  posted on the COA website.

Responsibility:

To fully and accurately complete the relevant application for (re)accreditation and return it to COA with all the required attachments in the specified timeframe.

Responsibility:

To "tell us how we are doing" by participating in COA's consumer organization satisfaction and opinion surveys.

Responsibility:

To fully and completely prepare a Self-Study document and prepare for a Site Visit using the current and applicable COA standards.  To work with COA staff  (Peer Services) to facilitate an on-site review.  This responsibility includes making all efforts to adhere to the (Re)accreditation Timetable created in collaboration with your organization during the Intake Phase of the  process.

Responsibility:

To adhere to the terms of  COA's Accreditation Agreement, and the Accreditation Policies and Procedures Manual.

Responsibility:

To promptly notify COA regarding changes to  your organization as it relates to leadership, physical address, delivery of services, mergers and acquisitions, and accreditation eligibility.

Responsibility:

After completion of the (re)accreditation review process, to maintain ongoing implementation of/performance with COA's standards and adhere to maintenance of accreditation procedures as described in COA's Accreditation Policies and Procedures Manual.
