Do you have a concern about a COA-accredited or in-process organization that you would like to share with COA? 

COA responds to all concerns falling within the scope of its standards. COA reviews concerns about its accredited or in-process organizations after receiving information from a stakeholder and a signed Letter of Intent/Consent. The consent gives COA permission to contact the organization about the concern and to disclose your name to the organization. 

How can I submit a concern?

If you wish to submit a concern, complete COA's Concern Form and Letter of Intent/Consent and attach any supporting or relevant information.

Mail completed forms to:

What if I cannot access the forms from COA's website?

You can request that COA send you these forms by:

         Emailing reportconcern@coanet.org, or

         Calling COA toll-free at 866.COA.8088 and asking to be connected with the Maintenance of Accreditation Coordinator at extension 285.

How does COA respond to a concern?

COA begins its review process upon receiving your Concern Form and signed consent.  COA may elect not to review concerns submitted: 

  • Anonymously, or 
  • Without a signed consent.

Actions taken by COA as part of its review process may include, but are not limited to  the following: 

  • Request information or written response to the concern from the organization.
  • Determine if a concern is standards related.
  • Maintain the concern in the organization's accreditation file.
  • Provide the concern to a peer review team for review during the organization's regularly scheduled site visit.
  • Conduct a site visit.
  • Refer the concern to COA's Accreditation Commission.
  • Change the accreditation status of the organization.
